Clinician Encounter / Office Visit

When the patient registers, the Hunger Vitals signs, 2 question questionnaire, is given to the patient. As stated elsewhere we incorporated this into a 7 question nutritional survey (see below). The questionnaire accompanies the patient to the exam room and is reviewed by the provider. Patients who were identified as food insecure are given a prescription for food and directions to the local food banks (food pharmacy's). This adds one to 2 minutes, at most, to the provider's time with the patient and can obviously be done by other office employees, if desired. (The patient's gratitude will be obvious!)

Typical Dialogue:

"I see at times you may have a challenge having sufficient food for your family. I?'m giving you a prescription and directions to go to the local food pantry where you can get food when you need it."
